We have a "Request Information" plugin on our two wordpress websites, Both are intended to collect simple form data and send it to an email address "info@....". One works, the other does not. The code behind the form is meant to be identical. I'd like the two forms compared to see why one works and the other does not. Also would obviously like documentation/recommendation on how to fix the broken one and, optionally, improve both.
