We have a web donation form that we would like redesigned. The existing form will be provided in html, which is also the desired output. The existing form can be seen here [login to view URL] The donation process is currently across three pages. The first page asks for the donor to select a program to donate to and asks for their details. The second page asks them to confirm. The third page gives them final instructions for making a donation via bank transfer. We are looking to completely remove the confirmation page from the process. We would like to split the first page into two pages, the first of which will ask the donor to select a program to donate to (with two new options, discussed below). The second page will ask for their details. Where possible, we would like you to improve the design and usability of the page with the goal of increasing donations. Two new options for programs to donate to are being added. The first is to give our organisation the choice of where a donation goes. The second is to donate to the Against Malaria Foundation. Clicking this button will take the donor to the AMF website via the following reference link ([login to view URL]). They will use the AMF website to make the donation. There are some other minor details which would be worth briefly explaining over Skype or similar at the start of the project. The html files for the current first and third pages are provided. The html for the current second page is not provided as it is not going to appear in the final version.
