Convert PDF form into fully functioning word document

We have a 40 page PDF form that we need converted to a Word document. The attached PDF form contains a variety of different field types (for example: dates, amounts, addresses etc.) Each field needs to be converted into a table in Microsoft Word so that data can be entered into them. Unless otherwise stated, we want the word document to appear identical to to the PDF, such that one cannot easily see the difference. As indicated in the attached instruction document, some fields will require reformatting. The instructions here are very clear, but please contact me if you are having trouble with a certain area and I will respond quickly. The deadline for this project is Monday, 2nd of June 2014 5pm AEST.
