ASP form to Database Table

I would like to have an online order form to be modified to dump information submitted into a database along with confirmation emails sent to both the user and admin. The order form can be viewed at: [login to view URL] This is the general format, but may need to be modified slightly to work within our current database structure. We currently have a working database that I would need a new table created to accept this data and also work in relation to the other table we currently have set up. I would also like to have a backend reporting system to be able to track and monitor the order. This can be a simple reporting system fot now, but will be upgraded in the near future. We mainly need to be able to track NEW/PENDING and COMPLETED aspects of the ordering process for now. This is the first phase of OVERHAULING our ordering system. We currently only use a FORMMAIL script from the order form, and it is becoming extremely difficult to keep track of the items order by our clients with only email tracking the orders.
