Clone input fields - Calculate total - Submit to database

This is for a webpage where the entry form must be separate and reset after clicking the -add item- button. The entries would then be copied into form input fields with unique id, totals need to be calculated and input values submitted to a mysql database. An image has been provided to give an idea what we are looking for. Thank you.

I apologize if my prior details where unclear, I am adding to my description to assist in understanding the project. Here is a little more information regarding the item entry form we are seeking to have completed:
- Items added should display similar to the format shown here.
- The "Add Items" form should reset upon adding a new line.
- Each item ID should be in numeric order - 1, 2, 3, etc.
- Each line should have the ability to be deleted and Item ID to reset.
- Each line should calculate its total.
- Total of all items entered should calculate in the Amount Total field.
- All items and amount total need to be added to mysql database upon submitting.

I can do it with Jquery and php, mysql. - Using Ajax call whenever user click add bottom - Then second Ajax call to retrieve data from database

You can use javascript to display hidden rows, or you can you javascript to to create new rows. The first one its more fast, but its limited to a predefined total.
