PHP form required for Wordpress Site

We are looking to get a form build on wordpress which is like a gift registry type on which they can enter details for 1-12 persons. We prefer to use Gravity form to make it possible and the payment has to go through [login to view URL] on checkout
If you look at the pictures there is one called [login to view URL] explaining how it should look like the actual form
[login to view URL] is demonstration showing different steps involved in making the submission
What we want is following
- User lands on form page
- 2 forms are automatically displayed once they fill form click add to list then automatically it converts into a card type of thing with message as per picture
- After converting to card if they click im done it will take them to checkout or cart then checkout and if they click add another it will open same form again and they can add new person in it
Please let us know if any one can do the task as soon as possible we will be hiring a programmer shortly as we have a tight deadline
