Need help implementing an affiliate script

I own a copy of Post Affiliate Pro, I can buy other scripts if the need be.

I would like for someone to help me implement this and make the script do the following.

1) Have a order form on my website something like request a quote form

2) When someone fills this form, his IP should be tracked

3) This IP address should be attached to the ID of my affiliate, who sent the visitor to my site.

4) Buyer pays me via Paypal or [url removed, login to view] or [url removed, login to view], then the affiliate should automatically receive commission.
