*** RUSH *** Minor Javascript edits/additions for order form

*** RUSH JOB ***

This is a fairly easy job, but we need it done immediately. This form is supposed to be up today (by the time our email newsletter goes out.) It is based on and should build upon previously written code.

We need some custom Javascript edited/written for our order form. It is partially working (using code from a donation form that AdamsK wonderfully coded). However, we need help to complete it.

1. User should be able to select any combination of t-shirt design sizes and quantities and have the total added and displayed in the "TOTAL AMOUNT" box. (And obviously, if user selects "qty" from a dropdown list after having chosen a number, quantity from that list only should be subtracted from the total amount.)

2. $5.00 should be added once for every order. This would simply be a fixed amount that will always be added... but, just once to the "TOTAL AMOUNT".

3. User should be able to input an amount in either or both of the "Additional donation" and "Special Mortgage Reduction Gift" text fields, and these amounts should be added to the t-shirt amounts and shown in the "TOTAL AMOUNT" field. (And, of course, if they delete or change their amount in these fields, the total should reflect that.)

4. The total amount is being saved in a cookie and is referenced on an "order-thankyou" page. This functionality (as well as all other existing form functionality) needs to continue to work as it currently is.

In other words, the form is basically just keeping a running total of any t-shirt selections made, along with any other donation amounts input by the user. This running total should then be shown in the "TOTAL AMOUNT" field. (Note: the form in its current form is already setup to send form results to the organization. This job is only for adding this specific JavaScript functionality to the form. Also, current cookie functionality of passing "TOTAL AMOUNT" to thank you page should remain functional.)
