Stripe Subscription Update Form

I currently have customers subscribed to monthly plans. When their credit card expires or if there isn't enough money on the card, then stripe automatically sends them an email to a link on my website with a form ([login to view URL])
The problem is:
When the user enters their details and submits it, stripe creates a new customer, which is creating duplicate customers ([login to view URL]). This also means that I need to manually re-subscribe them again on the 1st of the month.
The fix:
Instead, the updated credit card should be added to the existing user with the subscription ([login to view URL]).
Please describe in your own words how you intend on fixing this.
