WordPress Coded Client Triage System

I'm seeking a skilled developer for my current WordPress website. The roll involves coding a bespoke and secure online client triage system for our website to allow clients to self triage to the right psychologist depending on their selections/needs. Different questions will lead to different possible next questions (if/then rules)..
Responsibilities include:
- Implementing systems for client data input and storage.
- Creating triage assessment questions presented in an intuitive congruent with our online booking software (see attached screenshot).
- Building in functionalities to compile reports based on collected data.
- Ensuring it is customisable in the back end, as it will need to be edited as staff skills change and new members join the team.
Ideal candidate must have WordPress development experience, a strong understanding of data security protocols and prior experience with online form and report generation systems. Knowledge of HIPAA compliance would be an added advantage. This job requires creating a system that maintains privacy for sensitive client data and ensures only authorised individuals have access.
