Build an Online Survey for Data Collection
I need someone to convert an 80 question survey (Microsoft Word document) into an online survey. All of the content will be provided, so no copy will need to be written. Most of the questions are multiple choice (a-e), a few are true/false, and a few are short answer questions.
This script should have the following functionality:
1. Not Public. Only people who have permission should be able to access and complete the survey. This should be accomplished using a password that I would give.
2. For groups. This survey will be for companies. Some companies will have more than one individual taking the survey. The script would need to be able to keep individuals from the same company grouped together so the results can be aggregated later.
3. Reports. The script should be able to produce a report (and send it by email) which presents the results in two ways: The first would be to aggregate the answers of every person in the company and present them in a "comparison-style". The second would be the average answers of everyone in the same company.
4. I would need to be able to access the results at a single point in time. So the results would be saved until I decided to access them. Once all of the results were in, I could have the report sent to my email.
I also need this script to be fully commented so it is easy to edit in the future. I also need to be able to style and insert images for full graphic customization. This project needs to be completed in less than one week. It's VERY important that you be able to deliver soon.
