Custom FLV player

I need a custom FLV player built with my company logo somewhere in the skin. This will be used on my music lesson video? site as an embedded player. It needs to be able to accomodate chapter points and have chapter skip controls. Also I want a TABLE OF CONTENTS for other related videos built in to one side of the player, similar to the? same feature? on the [url removed, login to view] player. The player must be configurable to allow any size FLV.

I also need html coding and instructions on how to code my html pages properly, and upload the FLV player properly, so that anyone viewing my video FLV lessons gets the lesson loaded my my custom player, not some other? default flv player.
