Lead generator/pop-up for wordpress site

Hi,
We want to create a lead generator system on our website - performancedrive.com.au.
This involves creating a pop-up form that is easily configurable, asking users for simple details like email and phone number. We need to be able to edit the form, add colour and images if need be as well to make it look attractive.
I have already tried using a good system called Thrive Leads (Google it to check it out), but I couldn't get it working with our WP theme - it broke the website. Something exactly like that would be ideal though. (As an option, you can try to get the Thrive Leads plugin to work if you want to - I have it disabled at the moment).
Mobile friendly is a must.
Once the user inputs required fields, we need to have the option to put in an email address/es for the info to be sent to. Also need to be able to change timing of pop-up and frequency and so on.

Plugin developers and WordPress experience must be high. Not necessarily looking for quick/cheap fix, but it needs to be pretty simple to use and look decent on the front end.
