Script to process order

I need a simple form to be created where users can choose items from an existing application through a drop-down list. Upon submit, this form is sent to a payment gateway. User processes payment there and they are returned to our URL. Then a PHP script processes this successful purchase by sending a message to our application. I have all the details of API and sample script. Just create the form and make the process work as described
