Build an fillable form in Excel that transfers data to an Excel Table

We have a form that is currently in excel, but is not linked to a table. We'd like to redesign the form so that the data can be added to the form and then is automatically added to the table in a flat file format that can be uploaded.
The attached file has been unprotected so that it can inspected. Normally on the the data input fields are accessible. I'd also like to consider something that could be imbedded into an outlook email. I've also attached a file that illustrates the final format of the table to be created from the data filled into the form.
