I am looking to rebuild my site at:

to make it bigger, more user friendly and search engine friendly.
I have started a rough template design at:

The content of the site is my responsibility; I am looking for a designer that will build me an attractive, clean and smart template to use throughout the site.
I need the following:
* Website Logo (~310x50 see Notes.4)
* Template design (based on Fig.1 but 'prettied' up, see Notes.1)
* Small Signup form to go on template (See Fig.2, Notes.2)
* Big HTML signup form to go on separate page (See Notes.3)
The name of the new site is SMS Sex Chat and is aimed at UK visitors only. The purpose of the site is to get the user to signup for the mobile phone chat service.
Please only apply if you have lots of good feedback and examples of previous work.
I am not looking to baby-sit the designer through this process, the colours and image selection etc. is down to you.
I would want at least 5 different template designs and logos to choose 1 from that you will go on to make properly or modify with feedback from myself.
A Microsoft Word file has been uploaded with Figures and Notes, please do not bid until you have read through these and are happy that you can provide me with something that suit my needs to a high finished level.
Further work will be available to the chosen designer.
I believe this work could be done for a couple of hundred dollars - but I am looking for the best. I want a designer who has experience in making sites that convert surfers into sales. Getting the user to signup for the mobile phone SMS chat, that is the product.
