Create PDF file in PHP

An experienced PHP coder is required to code a single PHP file that would dynamically recreate the attached document, using the FPDF library.

It is of utmost importance that every aspect of the new PDF file closely follows the original. All of the document must be created dynamically - it must not contain static text that is represented by a bitmap.

FPDF library can be obtainded for free from www.fpdf.org.

The PHP file can be hard coded - no interaction with database is required.

The deliverables are: 1 php file uploaded to RAC.

The successful bidder is encouraged to carefully proof-read the final document. Sumbitted work that contains typographical or coding errors will not be accepted.

Please feel free to communicate and ask questions during the bidding process. Do not offer alternative solutions (e.g. different library etc.), as they will not be accepted.
