Build out website from complete wireframe and graphics

Looking for an individual/team that can DESIGN as well as PROGRAM that can build out a simple site that we have already created a wire-frame and (most) graphics for.
You will need to:
1. Build out site from the ground up based upon the wire-frame and guidelines provided
2. Upload site to a test server environment for us to test
3. Provide final build-out code to us
Site will include:
1. 15-20 separate pages
2. One page will need to include an interactive map that the user can search for a nearby location via their zip/postal code
3. Some pages will need to have automatically scrolling images
4. One page will include a fully functioning "contact us" fill in template that submits results to our corporate email
5. Site MUST be fully mobile ready
6. Site will be managed via Dreamweaver on our side
