Complaints form page with back-end

Development for the attached form as a normal HTML page, with a back-end to be able to:

- view all the complaints submitted in this form.

- Add cities to the last dropdown box.

- A place to add emails where they would receive the complaints on them.

As well as there would be a thank you message when people submit on the form.
