I have a job application form on one of my client's websites, which gets emailed to the owner upon form submission.
I have already created the application form ([url removed, login to view]), but the email format that gets sent is in the format:

So it's not easy to read. I just need it formatted so it's easy to read and doesn't have all the extra label junk.
I know it's an easy project, but I don't have the time to fix it myself.
I need someone to start this project immediately.
I've attached a copy of the form, along with the form sender script so you can see what I've got to work with. You're welcome to use any form mailing script that you want... I have no preference. I just need something that works.

I would suggest you to convert this to pdf format, so that it can filled and submit this to the client as shown in the format. Please let me know your thought.
Unless I run into issues, it shouldn't take more than a few minutes to take care of. If it takes less than 3 hours, (and I'm fairly sure it will take... like... 1 hour), I'll only bill $10/hour.
