Genuine designer needed for a \"simple\" website design modification PSD

Hello I hope it will be a pleasure working with you. I need an experienced graphic designer to do the following from an existing PSD website photoshop template design: 1.) Design a simple logo that will replace the demo design and match my website design. I have some pointers how it should be. 2.) Modify an existing PSD template - remove text/pictures+ edit color. This is a very simple website. 3.) Create a small "buy now" button & create a simple contact form design (name,email,phone, message) for 2.) - I also have examples of how both should look. I have the template files and have marked up exactly what I want modified from an example file. I have also made a short video for instructions so this will be a simple and smooth process for me and you. Please put words "intructions understood" in your bid so I know you have read & understood this. Please also tell me why I should hire you for this project and previous related work is a big bonus. Knowing English is also a big plus as in the future we could be using Skype on projects or so. If you exceed my expectations, you might be my new designer for future projects. Budget 120-150 USD depending on skill level.
