need .net electronic form for tablet use

need a electronic form that mirrors the attached form exactly. want users to be able to use tablet styles to input data, and have handwriting converted to text. must have a admin interface where drop down list can be modified as needed, and preform calculations. form must include a email submit button that also must save itself locally as a excel sheet. all source code must be provided, this could be an ongoing project for the right person
