Web App for entering data and importing in excel sheet

We are a team of developers with expertise and experience in developing web applications in PHP. We have coded PHP scripts to export/ import data from/ to database from excel. I have gone through the attachment. I am not sure about the fields for each tab - will they be same for health and all other tabs? Can you share fields for all tabs. The data showing on top is from the image uploaded by admin? Please share sample of image files. Please explain form type - H1 etc. mentioned in attachment. The project is very doable. Just need to be sure of finer points and need to assess scope of work and revise bid (proposed budget does not seem feasible). I ensure a very thorough job and also ongoing services if you need changes / enhancements in future.I look forward to hearing from you.
