Prestashop payment module small fix

I need someone who will adjust one credit card payment module. You need to add additional 2 select fields via smarty to the module. Design is finished, everything is up, module is working but we need 2 extra fields for customers to choose credit card types(amex, visa etc..) and installments (monthly payments 1,2,3,4 etc...). All other data is ready in the post form and module successfully redirects values to the credit card service. Only these 2 values are empty, we don't have them(cc_type, installments) . I need someone who can start right away. Thanks!
