On Apache 1.3.x, we need a couple of php functions that can be called from a form on an xhtml page requesting a login, in which the call to the php function passes login ID and a simply encrypted password. The reply from the php function must pass back parameters from a MySQL query using the login and user password and return fields for the form (name, address, etc.). User submission of the form must pass data to a php function that updates the MySQL database with a NEW contact record containing specifics from the form (requested service, date and time requested, etc.).
We also need a calendar pop-up for the xhtml form from which a user can select a date, and from which the form is populated with the date selected.
