Online booking and view booking with customized system.

I need a nice, easy and simple online booking platform that can be then use in different website of my business. So it's not just for one website but something that we can use/customized for different websites so that we get different data from different sites as a booking and from respective login ID, the booking is then view for respective sites. Following are the structure requirement. 1- Front booking : This is how normal customer/site visitor view while booking. Basically they can choose the date from calendar, and view available free time, then choose that time and book it by giving name, email, phone number etc through form. It's a simple. The only part that needs bit programming is to show the available free times. 2- Back part off booking( Viewing booking): For now we have simple mailing function when form is submit with php and it goes to the email. But now we want something more easy, the values goes to database. And with some login, those data can be seen in simple table design. Where site owner can see the bookings for today, tomorrow, and coming weeks if there's any. Also can see all the past bookings by clicking the calendar. So design can be like normal calendar of mobile with reminder, we can see whole month calendar, or weekly, or go to specific date and see the different times on that day. It should be real time( it's ok if it refresh autmatic in 2-5 minutes and display new data if there are any). 3- The main important part is, there should be some custom CMS kinda thing, so that if i want to add bookings to new sites, I can choose what are the value I want from form( like name, date, phone etc), and give the booking name, and get widget so I can put that in website I want, and in same time it creates new table for that specific site, which gives unique url, username,pass to login to that page, so each different site can view their own booking, from their respective URL. I am ready to clear any confusion. As myself a kind of programmer, I know this is not that difficult task. Just some database structure setup, and some coding. And it won't take long time. I can provide user interface design if needed, so you can only focus on database and coding part with basic css/html5. I will appreciate if someone want to work on monthly basis, as I have many project going on. I want someone whom I can trust, and do business together in future than just one project.
