Need help implementing wix feature ( find therapists by postcode and gender )

Hi. I need someone to create something like this: [login to view URL] I only need the searching facility, not the website designed. I need it to look the same as the link above with the same search options. Instead of "Enter Problem" I need it to be able to search by therapy type ( Indian head massage, back massage etc...), I want to keep the "Search by postcode option as well as the radius, but i would also like to also addan option to simply click the option to search for all of this by the gender of the therapist ( Male or Female ) I want it to show the results as pins on a map which shows the locations of the therapist's shop, preferably google maps. This is a simple job and may lead to more work, hence the low project value. I will need a mock up done before i pick the person to do this job!
