Building a mobile web application with a visual backend

I am looking for a developer who can make a little mobile web application. This application should be a contact form like the one from Airbnb where I attached a video below. The idea is to copy the visual and usability style of the Airbnb app but put it in a web application which works on ios and Android. I have also attached a file where I adjusted some details of how our contact form should look like. We are looking for a developer for all of our projects and future projects so this is more of a test run how it is to work on Freelancer on such project. The goal is also to build a visual editor of the contact form to quickly change the background color, the font color and the steps you can take. Another important step is to document the code very well, so every programmer who works on this project in the future can quickly understand it.
