Develop an online Form -Registration

We need a online form for www.gmcs.info.
The job is to create from our recent word form/catalog which contains 35 pages of text and fields to fill a online form that is accessible only by email and a password that we want to hand out. The data should be accessible sorted by clients/logins on a ftp and a backend login for us to review the filled info and documents.
Important:
its about 80-90 points that also need an upload function so that people can upload, images or pdfs etc as proof.
We need also a function to rate each point and that it calculates a total sum at the end or excel export of each ranking in regard to the fields/points
