I want you to create a script that takes an html form and fills a pdf form.
The script should reference fields in the html form and fields in the pdf form.
I want to use this script to use with any html form I create and be able to reference the fields in that form, then take any pdf form I create and fill the corresponding form fields I choose.
HTML Form I create (let's say 25 fields but structured so I can add as many as I want) => Database => fill fields in a PDF form I create (let's say 25 fields but structured so I can add as many as I want).

I really don't care how you do this, I just want to be able to fill a pdf form with the user input and I also want to pull data from a user data base to fill form fields as needed.
Attached is an example of the type of pdf form I want to work with.
I just want you to create the basic script. If I have the structure, I can add fields as I need to, this is not a specific form to specific pdf project, this is just creating the script or using a script you find or know of to allow me to map out my own html form to pdf form fields.
My Max budget on this is $100 and I don't think it should take that for someone who already is familiar with jsPDF or other pdf scripting knowledge and experience with databases.
