Create a method to have a member list in wordpress similar to example

I need to build into wordpress via plugin or otherwise a way to replicate a similar design of [login to view URL] Needs to have all the fields listed, name, category, primary contact, alt contact, and a details screen which can have an image, address, phone, fax, etc. Slight variation to the design is ok, but has to be pretty close. Needs to have a simple way to add people on the back end, via custom post type or WHATEVER. Time frame is pretty tight, so need someone who can get on this quickly. Must be available via skype.
