Create a Simple Wordpress Plugin

I need a wordpress plugin developer to create a simple wordpress plugin that can work with getreponse to create a form to collect data from visitors and save it in the database in wordpress admin area and be able to view all the details for each form and subscriber. The way it will work is that I will be able to click "Create Form" and go to a page where I can enter the name I want to give the form and also paste the getresponse code. Once I click "Create" button, the plugin will create the form and create the "Short Code" I can paste on a page that will now show all the option s of form field that I have created through the getresponse web form I created that produces the getresponse code I pasted. Then I will be able to click "View Form" and this will show a page that list all the forms I have created and displaying "#", "Form Name", "Short Code", "Download", "View Subscribers", "Date Created", "Edit", "Delete" as the headings. a. #: Shows number 1, 2, 3, 4, 5, .........., etc. b. Form Name: The name I gave to the form when creating it. c. Short Code: This is the code I will put on the page to displayed the form for visitor to fill out and submit it. d. Download: When the "Click" link under this heading is clicked, it will allow me to save the spread sheet that contains all the people that subscribed to this form to be saved to my computer harddrive or open them to view in spread sheet. e. View Subscribers: When "Click" link under this heading is clicked, it will show a page that is called "Subscibers List" and this will display all the subscriber with the following headings: "#", "Name", "Detail", "Date Submitted", "Delete". f. Date Created: That is the date I created the form. [login to view URL]: When the "Click" link is clicked under "Edit", this will allow me to change the name or paste a new getresponse code. h. Delete: This is to delete the form completely if I don't need it any more. Under the item "e" above, When "View" link under "Detail" heading is clicked, this will show a page that display full detail information or details of individual subscriber which is all the fields created through the getresponse web form. Please, view the attached spread sheet showing the sketch of the way the plugin will work. I need this plugin created, tested and completely functioning as soon as possible. So, tell me how long it will take you to finish, price, past experience and plugins you've created in the past.
