Calculate and display order total on web form

I have a page (see attached) to take orders for the service we provide. Customer enters name address and options with price, but the page does not display total

Customer places an order, then we get an email. That is good.
What we need is a total to be calculated and displayed on the page before customer clicks "Submit"
Total required for following (even if item value is currently 0)
+(A) Service Type
+(B) mail forwarding
+(C) Advance/Deposit
+ Tax 8.375% on A and B (not C)
Existing PHP File attached
Do not change any existing function like MimeMail or PDF file import
