Build a Website that will accept and manage applications

This spec is to build a basic site in PHP and MySql that will allow users to submit an application. The initial form will capture information like name, email address, telephone number, membership name and a few other fields to be confirmed. The user will need to create or be given a password so they can log back into the site to view the status of the application. As well as the user being able to login, there will also be staff members who will view and manage the applications. There are several staff members who each have a one or more geographical regions they manage. They should only be able to view the users applications that fall into their regions. The staff member should be able to add notes to the application, both private and shared. The shared notes will be viewable to the user, but private notes are only viewable to the staff member. The user should also be able to add notes in response. When either user or staff add a shared note an email should be sent advising of the update. Each application will have a status e.g pending, waiting these will be confirmed later. Only the staff member can change the status. When a status is changed an email alert should go to the user. The status should clearly be shown so the user can see the progress of the application. There will be approximately seven statuses. Once an application is accepted, it should be archived. When a staff member logs in they should first see an overview of all the applications in their region, and the current status. From there they can click an application to view in full and add the notes etc. as described above. As well as the applicant and staff member there will also be an Admin, the admin should have visibility of all applications and be able to perform the same actions as the staff member.
