Lost Sales Register

I require a small Visual Basic 6 program that will will link into an ODBC datasource called PPMPS and the database called PPMPS. The database should be in Access 2000 format. The application should be called "Lost Sales Register". The Visual Basic Front end should allow the user to record Customer Account Number, Part Number and reason for lost sale. This should be saved in a table in the database called lost sales. The Part Number field should be a List Box whos source comes from a table in the database called Part_Number. The part number table holds the part number and description. Once the user has selected the part number a text field should populate with the description. The user should be able to select the reason for the lost sale from a list box where the contents comes from a table called "reasons" in the database. For each record saved against a customers account you should be able to save up to 4 part numbers and the reason for the lost sale. A quick turn around would be appreciated. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased. ## Platform The front end will run on Windows NT 4 sp6 or Windows 2000. We require the application and the VB source code.
