WordPress data entry and results pages and underlying database

There are just three different types of pages involved. See both the sketches and spreadsheet documents attached. ** 1 ** Create data entry pages for WordPress site to be used by website user; 7 pages, up to 3 data fields per page; user progresses to next page by clicking Submit/Next button (see "Data Entry Pages" tab of attached spreadsheet). -- Each field is preceded by language provided in the "Field Description for User" column of the "Data Entry Pages" tab. -- Each page allows Buyer to add and edit text in WordPress -- PHP interface to/from the database -- Entries by website users will need error checking (did they enter the right form of information, is it within expected amounts, etc.) and notices to users about any error. ** 2 ** Create 2 separate data results / report pages for WordPress site, -- One covers data entered, formulas, results and calculations based on the user's entered data, and provides links back to data entry pages (see "Individual Results" tab of attached spreadsheet) -- Second provides line by line summary of data entered, results and calculations of 25 most recent users (see "Summary" tab of attached spreadsheet) -- Each page allows Buyer to add and edit text in WordPress -- PHP interface to/from the database -- Results pages must be printable by user ** 3 ** Create MySQL database/tables for storing and retrieving data entered by users; database on linux server; administered using PHPMyAdmin; each user set of entries uniquely identified by user initials and date/timestamp ** 4 ** In your bid, please tell Buyer about a project like this you've done in the past. ** 5 ** Deadline: 7 working days from day of award. ** 6 ** DO NOT APPLY FOR THIS JOB IF YOU HAVE NOT READ THE ENTIRE DESCRIPTION AND THE ATTACHED SPREADSHEET. ** 7 ** All intellectual property rights related to the project reserved and owned by Buyer.
