Auto-update script for Paysite tour

Write a simple script that will monitor a webform on server. Whenever an image is Uploaded to server at this location, the script must recognize this and update on the webpage with a thumbnail of the image found from the form upload.

Also display on the webpage "Added 2-22-2014" underneath the thumbnail

This is fairly simple, you must demonstrate these functionalities in Jquery, mysql, javascript, PHP, ASP, java etc

Portfolio or resume included in Bid proposal gives you priority.
