Re-write MS Acess app in php/jscript/Ajax/MySQL: phase 1

This is a simple startup phase to find the provider who will complete the whole project. This first phase comprises: 1. Build a basic menu structure containing 5 items each on two levels. 2. Convert (from MS Access) or create a client capture form. 3. Convert (from MS Access) or create the Client table in MySQL. 4. Write logic to construct a new client code from information entered by the user. 5. On the client capture form present a small sub-menu containing ten items. This is not more than $30 worth of work for an experienced provider. If you bid more than $30 please justify the additional amout of your bid otherwise you will definitely not be chosen and your bid will be wasted. You will work on my server so that I can monitor progress. I am looking for someone who communicates well and is flexible enough to work with existing code that I have already.
