PHP upload form with additional fields

I need a PHP page created with an "Upload Files" button at the top. When the user selects which files to upload, the upload should begin right away and a progress bar should appear. Only certain file types should be allowed in the upload (MP3, WMV, MP4 for now but I should be able to customize this). There should be another column with two check-boxes. Once the download completes, I need two other columns to become populated automatically based on a calculation. The second column should change values dynamically based on the two checkboxes mentioned earlier. There should also be a delete button next to each uploaded file to remove it. After all "rows" with files, there should be another row showing the total of the values in the second and third column so I need some variables that get updated each time a file is selected to upload. All this should happen on the same page, not being redirected to another page. I'll send a mock-up of what the screen should look like but the details above should give you everything you need in order to bid. Let me know if you require any further details. I can also provide a URL that shows this in action, or something close to it that I've been able to find. I don't want to use tables. Please use DIVs instead.
