I need my (already written) free eBook set up to collect names and email addresses

I wrote an e-Book, which I intend to distribute for free. I also intend to password protect some of the pages so that when a user hits a password protected page the book will ask for first, last name and email address. The book will request a particular password from a pool. I need the form set up in the book and a script or software to accept the request from the eBook and email the needed password to the user. I already have a form on my web site [login to view URL] and to collect emails I use MailList Controller software. The actual eBook was created using the Activ Ebook Compiler by [login to view URL] A fully functional free version is available for download. Some of the features are locked. In sumary, I intend to use this e-Book to collect information for my opt-in list. Scripts can be installed in the book or on my web host server that supports MySQL database. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform The e-Book requires an installed Internet Explorer. Avtiv Ebook Compiler is an HTML compiler and allows for scripts to be installed and this way built in the completed e-Book.
