I have a form I wrote in PHP with a little Java.... you can find the form at [url removed, login to view] My problem is i want the form to show up prior to the select a campus drop down option...... I would like a form that has the same fields i have now in this order: 1: firstname 2: lastname 3: address 4: city 5: state 6: zipcode 7: dayphone 8: eveningphone 9: cellphone 10: email 11: age 12: campus and program area - which when you select one of the campuses, it opens up a drop down menu below it...which depending on the campus you select...it has different programs... i.e. u select Union - Automotive is a program, Nashville - automotive, diesel, collision, and high performance are programs...... 13: Have you graduated High School - which has a drop down - Yes - No - Still in Highschool - When u select Yes...there is a dropdown that says Year graduated highschool with a dropdown list of years from 1960 - current year (dynamically generated). If you select No, there is another dropdown stating GED: with years ranging from 1960 - to current.... If you select Still in Highschool - there is a dropdown that says Year of Graduation: With dates ranging from current year....to 4 years in advance..current year, current +1, curent +2, current +3
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
