Php / mysql two questionnaires, tables created, partly done.

I'm looking for a FL to complete a partly developed questionnaire system. You are expect to build 2 questionnaire forms of about 35 questions each, with functionality like update, and continue editing, view the questionnaire etc. (both questionnaires are the same but questions in different order.) The data table for this already created, there is a an HTML version as well. You may have to re format and introduce the php table updating and validating functionality. Please find the enclosed document. I have described the project in detail. Please make sure you read it properly. I need this done asap, I want to deal with actual programmer, not with a company.
