Write an iPad application

Looking to make an app for a yearly subscription item that would include a way to scan a bar-code that goes along with each particular subscription.
Each customer would have to read, fill in their name, email address, phone number, initials, and sign an iPad page of terms and conditions (in person). That information would then be emailed to the customer and the clients email address.
That new account that was created would be assigned a bar-code that would be scanned in at the time of purchase (a tag that they would get and keep, which is a bar-code with a 12 digit number). This would also be emailed to the customer and client along with that original email.
App would need to not depend on internet since this may sometimes be done outside of internet access. So, that information would need to be stored and then, when connected to the internet again, upload.
The bar-code would be captured using the camera of the iPad.
Client has multiple locations so these would need to be installed on multiple iPads (about 15 to start out). This is a strictly internal company app so it would not need to be sold through the Apple store, etc.
