Build an app

I require an easy to use app to be built to make my job easier for roof inspections. 1. I require a home page where i have address and take photo of front of house 2. I require a menu with approximately 10 sub headings. i am able to pick as many of these as i require 3. each sub heading has approximately 20 questions where i can answer yes/ no or N/A if i choose N/A i go to next question. If I choose yes or no i can take 1 or multiple photos and draw arrows or circles on these photos in multiple colours. Next to every photo i can write a heading and TEXT. 4. Because most of what i write is the same I need the ability to have pre written TEXT answers that i have saved also the ability to use pre made photos or diagrams At the end the answers to the questions/ photos and text need to be presented as a report. Anywhere where i have answered N/A the question does not come up in the report. The report needs to be in word format On the keyboard function I need a seperate pre written NOTES function where I can choose pre written information There will be 3 headings that if i choose pre written NOTES it will auto fill information to a different heading as well I would like the ability for my admin to be able to contiunually add notes to the system without my employees being able to change them in the back end only me once form is completed by staff I need them to press submit and it comes directly to my admin. I would like the ability to change wording and mark up photos that an employee has done in the field
