InDesign Name & Address Merge -- PLEASE READ CAREFULLY

Hi! The project will consists of a base project, plus 1 option that will need a separate price quote. The base project: 1) Make a few changes to the 1st page of an Illustrator document. 2) Take the 6 page (3 front pages & 3 back pages) Illustrator document and convert it for use in InDesign. USER should be able to edit in InDesign. 3) Enable the USER to upload a name (or names) and an address (but sometimes no address), so that the 1st page of the document contains a message that includes this information. See attached image files. - The end result will be an InDesign files that contains a different name (or names) and possibly an address for each of the 1st pages. - This base project has a budget of less than $100 Option (please price separately from the base project): - This option is to make a macro so that the base project doesn't need the user to input the information into a Excel file. - Please let me know how much extra is would cost to have this function. Other requirements: - The print resolution of the finished product must be as good as the original Illustrator file - The USER must be able to print all 6 pages at once (a single document) with the 1st page having the information (name and address) merged. - The name, address, and message on the 1st page should be a font that looks like a handwritten script. Please let me know if you have a few fonts to choose from (other than what is already available from Adobe). Thanks for bidding!
