design ideas as well as PHP form fixes, 2 freelancers to be selected

PLEASE DO NOT BID IF YOU HAVE NOT READ THE DESCRIPTION AND TESTED THE FORM hi, we have a form built from previous developer, which we didn't liked and also it does not work well. for this project we will hire 2 people sepearetly, one will b designer and other will b programmer to make that design of form. this is the form made in PHP-AJAX (i think): [login to view URL] these r the tasks to b done from diff freelancers on same form: FOR DESIGNER: 1. we need to have 2-3 form designs, more fields than this one already told 2. test the form and see how confirmation message looks, this is very ugly, but we want same msg to print with a good design FOR PROGRAMMER: 1. making forms and confirmations as per the designer we will select in same project. there will be 2-3 forms with more than 7 fields but within 20 fields. 2. if u test current form, we have a email and SMS notification when someone submits, but once submitted sometimes the confirmation does not work 3. after submitting the form once, the form does not accept second submission, dnt know y, u need to correct this issue. for this plz test the form more than 1 times LOWEST BIDS WIN. GENERIC BIDS WOULD BE IGNORED. IF U WANT YOURSELF BE CONSIDERED. PLEASE TELL US ATLEAST ONE ERROR MSG U GOT WHILE SUBMITTING FORM.
