Touch Screen Door Prize Kiosk Entry Form - Update B

Hello, We would like to update our Kiosk Software again. : ) 1. we would like to change the wording "Would you like to receive emails from our Scrapbook Partners?" to "Would you like to receive Deals/Tips from Weekly Scrapper?" 1a. Change the checkbox to be checked by default. 2. We need to add a "Which Days will you be attending" section that gives the choice of Thursday, Friday, Saturday and Sunday. They can choose any or all days, and ideally have it auto select Friday if the current day is Friday, or Saturday, etc.. (We also need the ability in the Admin Control Panel to hide/unhide Thursday and Sunday as some shows don't have these dates.) 2a. When drawing a name at random we want the computer to only pull names based on the day. So if we draw a name on Friday we only want people who selected "Friday" from above. That way if someone only came Thursday we are not drawing their name for a prize they will never pickup on Friday. 3. When drawing a name at random we would like to display the city and state based on the zipcode. I have purchased a Database of current City, State, Zip so a simple query of the Zip Code should return the associated City & State. This is so when we draw a name for John Smith and we get 12 showing up we have an issue. However if we say John Smith from Puyallup, WA then it narrows it a bit. I can send you the Table Structure for the ZipCode Data we have purchased, if that will help. Example: {Need to take into account all zip codes need to be 5 characters long so you may have to add Zeros to the front of a 4 digit zip code.} ZipCode.....|.....City.....|.....State.....| 00501........|..Holtsville.|......NY.......| 47031........|....Milan....|.......IN........| 4. When drawing a name at random sometimes the system hangs and does nothing. We have found that on occasion one of the stations is not responding but we have no way to know which one. So we end up checking each station to make sure the Web Server and Database Server are running, etc.. Is it possible to when clicking a button like Select Random Name (or other buttons that access the other kiosks) to have it display "Kiosk-1 | Done" "Kiosk-2 | Done" "Kiosk-3 | Pending", etc. so when it hangs we can see which station it's looking to and can't access. So we can go to that computer and fix the problem. 5. Add a section "Where did you hear about us" with the following choices. a. Facebook, b. Friend, c. Scrapbook Magazine, d. Prior Show Attendee, e. Michaels Craft Store, f. Other (with a box to specify what other.) 6. Finally we would like when a entry is submitted that in the background it sends a email (through our SMTP Email Server - So Outgoing Server, Name & Password would need to be editable in the Admin Pannel Somewhere - password ****ed out ofcourse.) to the email address (using their full name) with a Subject & HTML Body that we put into the system somewhere. Enter it all in a .txt file on the computer or enter it in the database somewhere. Where-ever you think would be best/easiest for you and for us to edit the email on occasion. (Example of email: To: Taybor Judd From: Scrapbook EXPO Subject: {Editable} Body: {Editable HTML Body} I think that's it. : )
