Hello, I have seen your attachment and got a form in which details would be entered to be forwarded to workshop controller. My understanding on it says that the app will have this form which will be filled by the user (Truck driver, Truck Owner or any other) and it will get submitted to your workshop (may be on your mail address or through sms notification, as suited to you) Let me know if I am going correct or else please correct me!!
