A wordpress plugin, or mod, or just a way to use an existing plugin.

I am in need a wordpress plugin or mod. You could also implement another plugin that is already out there. I just can't find one that I would know how to implement this. I tried "advanced custom fields" but a little over my head.
I am running wordpress 4 with wishlist member plugin.
Here's a description of what I am looking for:
Imagine I am a user that logs into the membership site.
I click on a post. There is a checklist per post that appears with predefined choices. The user can check one of the boxes, or multiple and it is saved in the user database per user. The choices that were made appear everytime for the user when he is on that post. It is a way for a user to keep track of what they have done per post. ie. On the post there might be 4 exercises they have to do. THe user clicks them when he completes them. Nothing needs to happen with the data, it just needs to be displayed and saved for each user.
On the admin side, when a post is created, I want to be able to define the items in the check list. Some will have 3, some will have 10, other's will have 15.=, etc. I want to be able to use a shortcode to display the check box listing where ever I want on the post page.
