Database from Excel form along with reports

I need to create a database that is derived from an excel sheet. All parameters should allow free text entry and some are fixed drop down menus with the option of entering free text. at the end the data entry user should have the abilit to print a specific hard copy of the report along with a soft copy. Some fields may contain additional information for the data entry user, these information assist the user in entering the required data, they should appear once the mouse is placed over the field (see the excel sheet) I would also like to have the ability to import and update my database. I also need an option, where if I click a button, an option to send an email (would be using outlook as mail client) should come up and I should be able to send the email. This project should be delivere as opened, thus having the ability to edit the (features, formats, settings, etc.) if required.
